I. Insurance Overview

(1) Insurance Provider

  1. For Academic Years 115–116, the student group insurance is underwritten by Farglory Life Insurance Co., Ltd.
  2. For Academic Years 113–114, the student group insurance was underwritten by Mercuries Life Insurance Co., Ltd.

(2) Insurance Period

  • First semester: August 1 to January 31 of the following year
  • Second semester: February 1 to July 31

※ The insurance coverage period includes both winter and summer vacations.

II. Insurance Claims

(1) Required Documents

  1. Insurance Claim Application Form (The form may be obtained from the Health Center or downloaded from the Health Center website.) The form includes the Consent to Inquiry and Authorization Statement, which must be affixed with a seal.
  2. Original or Copy of the Medical Diagnosis Certificate (A copy must bear the hospital’s official stamp.)
  3. Original or Copy of the Medical Expense Receipt (A copy must bear the hospital’s official stamp.)
  4. Copy of the Cover Page of the Applicant’s Financial Institution Passbook. (The account holder’s name and account number must be clearly shown.)
  5. Medical Imaging CD for Fractures. (If a fracture is diagnosed, please submit the relevant Medical Imaging CD.)
  6. Proof of Tuition and Miscellaneous Fee Payment. (Please log in to the Portal System and select:Student Services → Student Financial Aid Services → Print Tuition and Miscellaneous Fee Payment Certificate )
  7. For International Students:Please submit a copy of both the front and back of your Alien Resident Certificate (ARC).
  8. For Applicants Under the Age of 18 (The applicant’s legal representative must provide their national identification number and sign or affix a seal. A copy of both the front and back of the legal representative’s identification card must also be submitted.

※ Claims must be submitted within two years of the accident date. Please apply early to avoid delays or missing the deadline.

III. Enrollment, Declining Coverage, and Withdrawal from Insurance

(1) Application Information

1. Insurance Enrollment for Students on Leave of Absence

Students on leave of absence who wish to continue their insurance coverage must apply for an insurance enrollment certificate at the Health Center within seven days after the beginning of the semester and complete the required enrollment procedures.

2. Declining Insurance Coverage for Enrolled Students

Enrolled students who wish to decline student group insurance coverage must download the Student Group Insurance Coverage Declination Procedures and Declaration Form from the Health Center website and submit the completed form to the Health Center within seven days after the beginning of the semester.

3. Insurance Withdrawal for Students Who Lose Their Student Status

Students who withdraw from the University or otherwise lose their student status must submit an insurance premium refund application to the Health Center within seven days after losing their student status.

4. Students Who Take a Leave of Absence During the Semester

Students who take a leave of absence during the semester will not receive a refund of the insurance premium. Their insurance coverage will remain valid until the end of the insurance period for that semester.

IV. Frequently Asked Questions (FAQ)

Q1. Can I apply for insurance benefits if an accident occurs off campus?

A: Yes. You may submit a claim as long as the accident occurs during the insurance coverage period and meets the requirements of the insurance policy.

Q2. Can I apply for insurance benefits if I am injured during winter or summer vacation?

A: Yes. The insurance coverage period includes both winter and summer vacations.

Q3. Is outpatient treatment covered?

A: Coverage is subject to the terms, conditions, and claim regulations of the insurance policy for the applicable academic year.

Q4. Can students who have exceeded the standard period of study participate in the student group insurance program?

A: Yes. They may enroll in the insurance program and pay the required premium in accordance with University regulations.

Q5. Can students on leave of absence participate in the student group insurance program?

A: Yes. They must complete the insurance enrollment procedures within the designated application period.

Q6. How long does it take to receive the insurance payment after submitting a claim?

A: Processing time depends on the insurance company’s review procedures. It generally takes several weeks to approximately one month.
