Student Group Insurance

1. Starting from the 2020 academic year, the student group insurance is underwritten by Mercuries Life Insurance. For inquiries regarding student group insurance, you may directly contact the group insurance claims service representative from Mercuries Life Insurance.

2. Insurance Claims Service Consultation Hours:

• Every Tuesday from 11:00 AM to 1:00 PM
• Location: Health Center

3. Required Documents for Claims Application:

• Original or copy of the medical certificate (copies must have the hospital's stamp)
• Original or copy of the receipt (copies must have the hospital's stamp)
• A photocopy of the cover of your bank passbook (including account name and bank account number)
• Insurance claims application form (available at our office or downloadable from Health Center website)
• If a fracture is involved, please include the X-ray CD
• For those under 18, the legal guardian must provide their ID number and sign or stamp the form
• Proof of tuition payment (access via the Portal system -> Student -> Life Services -> Proof Payment of Tuition)

4. Once the required documents are prepared, please submit them to Health Center for verification during office hours.
