Instructions for Freshman Health Examination

In accordance with the School Health Act and the Regulations Governing the Implementation of Student Health Examinations, National Central University conducts student health examinations to safeguard students’ health, enable early detection of diseases, and prevent the spread of infectious diseases on campus. Relevant regulations are hereby established (see Appendix 1 for details). Students may collect their student ID cards only after completing the health examination.

Important Notes

Overseas Chinese students and international students must complete the freshman health examination in addition to the health examination required for residence permit application.

Labor physical examinations or military service physical examinations do not meet the Ministry of Education’s requirements and cannot be used as substitutes.

Students who fail to complete the freshman health examination will not be able to complete registration verification or collect their student ID cards.

I. Methods for Freshman Health Examination (Choose One)

On-campus Health Examination

Freshmen may participate in the on-campus health examination held in August or September, according to the schedule announced by the University each year.

Off-campus Health Examination

Students who are unable to attend the on-campus examination must complete the examination at a qualified medical institution before the start of the semester.

The health examination report must be issued within three months prior to the start of the semester

(e.g., if the semester starts in March 2026, the report must be dated after December 2025).

Examination items must comply with the University’s requirements.

A completed Health Information Card must be submitted together with the report, and all required examination items listed on the card must be completed.

Registration verification and student ID card collection are permitted only after confirmation by the Health Care Section.


II. Submission of Off-campus Health Examination Reports

University Partner Medical Institutions

(Taipei Qixin Clinic; Zhongli Tien-Sheng Hospital)

Bring the Health Information Card to the medical institution for the examination.

After completion, the medical institution will forward the examination data to the University.

Please proactively submit the examination receipt (in person or via email to ncu7270@ncu.edu.tw).

Appointment booking, report inquiries, and applications for paper copies are handled according to each institution’s regulations (see Appendix 2).

Non-partner Medical Institutions

Submit the health examination report and the Health Information Card (with photo affixed and signature).

Reports may be delivered in person or by mail during office hours to the Health Care Section, Office of Student Affairs, or submitted via email.
