All faculty and staff (including full-time and part-time instructors) are subject to all provisions of Occupational Safety and Health Act. To prevent occupational hazards and ensure worker safety, employers must conduct the following health examinations annually as per Article 20 of Occupational Safety and Health Act:
1. General health examination.
2. Special health examination for those engaged in work with specific health hazards.
3. Health examinations for specific targets and items designated by Central Authority.
Workers have an obligation to undergo the examinations mentioned above. Non-compliance may result in penalties.
I. Legal Basis
1. Occupational Safety and Health Act.
2. Enforcement Rules of the Occupational Safety and Health Act.
3. Occupational Safety and Health Education and Training Rules.
4. Regulations of the Labor Health Protection.
II. Penalties
Violations of Article 20 of Occupational Safety and Health Act may result in fines for employers ranging from NT$30,000 to NT$150,000 as stipulated in Article 45; employees may face fines of up to NT$3,000 as per Article 46.
III. Health Examination Items
Health examinations are divided into general and special examinations. All faculty and staff must legally undergo general health examinations. Each unit must confirm individuals exposed to specific health hazards according to the statutory regulations for special health examinations and provide a list of those required to be examined based on the working environment and materials.
1. General health examination items are specified in Appendix 9 of Regulations of the Labor Health Protection.
2. Special health examination and health examination items are specified in Appendix 10 of Regulations of the Labor Health Protection.
3. General labor health and health examination records are specified in Appendix 11 of the Labor General Health and Health Examination Records.
IV. Health Examination Schedule
The health examination items must comply with legal regulations, according to Article 17 of Regulations of the Labor Health Protection:
1. Individuals aged 65 and older should be examined annually.
2. Individuals aged 40 to under 65 should be examined every three years.
3. Individuals under 40 should be examined every five years.
4. Those engaged in work with specific health hazards should undergo a special health examination annually. (According to Article 18, point 1 of Regulations of the Labor Health Protection: Employers must conduct special health examinations annually or whenever work changes for employees engaged in specific health hazard work as defined in Appendix 10 of Article 16.) The general health examination items and records specified above should be conducted according to Appendices 9 and 11. However, individuals diagnosed with congenital color vision deficiency are exempt from further color vision testing.
V. Cost of Health Examination
The cost of the health examination is covered by National Central University.
1. Faculty and staff over 40 years old may apply for reimbursement of up to NT$4,500, limited to one examination every two years (as announced by the Personnel Office).
2. Those who cannot undergo the examination on campus must pay for their examination at an hospital (please refer to the designated medical institution website for labor health examinations: https://hrpts.osha.gov.tw/Home/CertifiedHospInfoSearch](https://hrpts.osha.gov.tw/Home/CertifiedHospInfoSearch) and submit the examination report to Health Center within one month of the examination date.
VI. Health Examination Hospitals

VII. Health Examination Questionnaire
In accordance with regulations, during physical examinations, faculty and staff are required to complete the "Ergonomic Hazard Prevention Assessment Questionnaire", "Overwork Assessment Questionnaire" and "Work Ability Assessment Questionnaire" to identify high-risk employees early and ensure timely intervention by occupational physicians.
