Faculty and staff (including full-time and part-time instructors) at our university are subject to all provisions of Occupational Safety and Health Act. Upon reporting for duty, they must submit the following documents: "General (Special) Health Examination and Training Notification", "Occupational Safety and Health Education Training Course Certificate," and complete the "General Labor Health and Health Examination Record." Additionally, they must submit either a "General Health Examination Report" or a "General + Special Health Examination Report." Those who fail to comply may be subject to penalties.

I. Legal Basis

1. Occupational Safety and Health Act.

2. Enforcement Rules of the Occupational Safety and Health Act.

3. Occupational Safety and Health Education and Training Rules.

4. Regulations of the Labor Health Protection

According to Article 20 of "Occupational Safety and Health Act" and Article 16 of "Regulations of the Labor Health Protection", employers must conduct Health examinations when hiring workers. Employees have an obligation to undergo general health examinations. Occupational Safety and Health Act mandates the responsibilities of both employers and employees regarding the Health examination of new personnel. Its purpose is to identify the fitness for work of employees, assess their suitability for the job, and prevent health threats or injuries resulting from work. Therefore, the Health examination must be completed before the employee begins work to fulfill this purpose.

II. Penalties

Violations of Article 20 of "Occupational Safety and Health Act" may result in penalties for employers as stipulated in Article 45, with fines ranging from NT$30,000 to NT$150,000; employees may face fines of up to NT$3,000 as per Article 46.

III. Health Examination Items

The Health examination is divided into general and special examinations. Faculty and staff must undergo a general Health examination. The employing unit must confirm whether new employees are exposed to special health hazards and implement the necessary statutory special health examinations and general health examinations based on their work environment and materials.

1. General Health examination items are specified in Regulations of the Labor Health Protection- Appendix 9.

2. Special Health examination and health examination items are specified in Regulations of the Labor Health Protection - Appendix 10.

3. General labor Health and health examination records are specified in the Labor General Health and Health Examination Records - Appendix 11.

  • If the general health examination occurs within the periodic examination period outlined in Articles 17 or 18, workers may be exempt from undergoing it, provided they submit proof (e.g., the health check report for new students at National Central University; they must fill out the "Personal Information Request Form" at Health Center). The health check report can be submitted upon reporting, skipping the need for a health exam.
  • The general health examination items and records should be conducted according to the requirements of Appendices 9 and 11. However, individuals diagnosed with congenital color vision deficiency are exempt from further color vision testing.

According to Article 16 of “Regulations of Labor Health Protection”, individuals are exempt from the general Health examination if any of the following conditions apply:

1. Non-continuous temporary or short-term work lasting less than six months.

2. Other regulations that already specify Health or health examination requirements.

3. Other exemptions designated by Central Authority.

IV. Validity of Health Examination Reports

The Health examination items must comply with legal regulations. The examination report's validity must meet the requirements of Article 17 of Regulations of Labor Health Protection:

1. Individuals aged 65 and older should be examined annually.

2. Individuals aged 40 to under 65 should be examined every three years.

3. Individuals under 40 should be examined every five years.

V. Cost of Health Examination

The cost of the Health examination shall be fully borne by the faculty and staff.

VI. Health Examination Hospitals

The Health examination must be conducted by a physician at a medical institution recognized by Central Authority in consultation with Ministry of Health and Welfare. Please refer to the designated medical institution website for labor Health and health examinations (all hospitals listed are acceptable).

VII. Reporting Instructions

Before reporting, new employees must undergo a general Health examination at a designated medical institution for labor Health and health examinations and submit the completed examination report to Health Center. (The examination process at the hospital takes 14-30 working days, so please start the process early to avoid affecting the completion of the reporting process or facing penalties; any related responsibilities shall be borne by the individual).

VIII. Health Examination Guidelines

1. Please adhere to the legal examination requirements and conduct the Health examination at an accredited medical institution.

2. On the day of the examination, wear comfortable and loose clothing, and fast for at least 6-8 hours. Avoid strenuous exercise the day before to ensure accurate test results.

3. If you wear glasses, please bring them on the day of the examination for measuring corrected vision.

4. For a chest X-ray, wear a top without buttons or metal accessories. If you are pregnant, please inform the staff before the examination.

5. For urine tests, collect a midstream urine sample. If you are on your period, please inform the testing personnel before the examination.
